Nursing Scope of Practice is a set of rules/laws and regulations that nurses are required to follow while practicing as nurses. These rules/laws and regulations help guide our practice and help protect the patient as well as the nurse. Each state has its own laws and regulations according to licensure type. According to (“What is scope of practice?”, 2022) “Scope of Practice refers to those activities that a person licensed to practice as a health professional is permitted to perform, which is increasingly determined by statutes enacted by state legislatures and by rules adopted by the appropriate licensing entity”.APA , Nursing

As nurses we are responsible for practicing within our scope of practice, knowing how to access our scope of practice (Nurse Practice Act or BRN), and continuing our education to keep up with changes that occur in the current scope of practice so that we can provide our patients with safe and quality healthcare (Berg, 2022). If a nurse is asked to perform a duty/task that is not within his or her scope of practice the nurse should advise the practitioner/manager that they are unable to perform the duty/task as it does not fall within their scope of practice. If the nurse is unsure if the task falls within their scope, they should refer to the nurse practice act or the state board of registered nursing to review their scope of practice.APA , Nursing Accepting or performing a task that is not within the nurse’s scope of practice can lead to patient harm as well as serious consequences for the nurse. Nurses who practice outside of their scope can lose their employment, face punitive damages, and possibly lose their license (Berg, 2022). As nurses, our main goal and focus should always be the patient’s safety and providing them with quality care. All healthcare providers should follow their scope of practice to provide the patient with the best possible care and enhance patient care outcomes. In my current position, I am employed as an LVN, and on several occasions, I have been asked to perform RN duties by other RNs, I just kindly remind them that I am employed as an LVN, and I am not allowed to perform RN duties at this time.

The plan to tackle the issue of non compliance to HIPPA, regarding patients having access to their own patient data, included questionnaires to gather where breakdowns occurs, and attacking those areas of weakness to remove barriers and realize compliance. “The study revealed that there are discrepancies in the information provided to patients regarding the medical records release processes and noncompliance with federal and state regulations and APA , Nursingrecommendations.” (Lye et al., 2018). In the plan we identified and pulled together a team who’s members are in the position to analyze the data, and then have the authority to enact policies. Educational needs were identified, and the managers needed to create, deliver, and assess staff’s knowledge base are also included on the team. The questions used in the exit questionnaire to identify and resolve non compliance are an excellent Quality Improvement tool, as they provide an ongoing quantitative and qualitative surveillance of the issue. As the policies are placed and staff educated, new exit interview should show a systemic progression toward compliance, and identify new challenges that require attention. APA , Nursing

This would be an excellent tool to use where I work. I, myself, am not clear on the procedure of where to direct patients for their records. Furthermore, with facilities having 30 days to provide records, patients wanting to review medical information during their hospital course to aid in decision making is not addressed. I would include in facility policy that includes the HIPPA requirement, of providing records within 30 days, and include a flagging mechanism for patients who need the information on a more urgent timeline.APA , Nursing

What factors do you see in practice that influence safe, quality, patient-centered care?

Multiple factors are driving or influencing safe, quality, and patient-centered care. They include communication, technology, shared decision-making, collaboration, and policies/regulations/laws (Mosadeghrad, 2014). For instance, technological changes are pushing the creation of new health IT systems like electronic medical records (EHRs) in clinical settings to improve the quality and safety of care. 2. Have those factors shifted or changed as your role as an RN has progressed or changed?

I strongly believe that the five factors have significantly shifted with the advancement of my role as a registered nurse (RN). For example, technology, communication, collaboration, and shared decision-making have become key factors driving my work, especially as I transition from a mere bedside nurse to a clinical nurse coordinator. I rely on these factors to supervise patient care transitions, conduct informatics analysis and design, coach patients, and manage population health. The fifth factor (policy, laws, and regulations) has also shifted as I have developed policy advocacy skills. I now understand my role in policy, especially in changing practice and professional laws, policies, and regulations to improve quality, safety, and patient-centered care.APA , Nursing

3.Do you think the factors will continue to change?

There is no doubt that most of these factors will continue changing from time to time. For example, new technology and innovation will continue being developed, and the role of RNs will change to match the new technological requirements. New policies, regulations, and laws will also continue to be passed or amended at the federal, state, and local levels. These new policies will stipulate the new roles of RNs in upholding the quality of care and patient safety. APA , Nursing

4.What factors do you see affecting safe, quality, patient-centered care in 10 years?

In ten years, the factors I believe can affect quality, safety, and patient-centered care are the transition of care to telehealth services, a shift in healthcare systems, the integration of IoT-enabled devices, and changes in law and policies (Zimlichman et al., 2021).

I have noticed that organizations who underperform financially may struggle to maintain quality care and patient safety, thereby resulting in less patient centered care due to compromises they make to be able to afford to keep the lights on. Hospitals who are able to perform financially have more freedom to invest in various quality improvement projects. This can result in safer quality care, setting them apart from other institutions which perpetuates their image as an organization in which patients can rely on for their care and bringing in more patients.APA , Nursing

As I have advanced in my career I do not thing these factors have changes in any way. I do think that they affect organizations because nurses do not want to work in organizations in which safety and quality have been compromised out of fear of loosing their licenses. Better performing institutions can afford to pay nurses a more competitive wage, makes them more desirable and pulling in more experienced nurses.APA , Nursing

I do not think these factors will change in the future. I agree with out textbook when it states that part of the issue is that there is not enough well qualified nursing leaders at decision making tables throughout organizations to bring up safety and quality issues (Sherwood & Barnsteiner, 2017). Organizations who are struggling may benefit from brining in highly respected, experienced nursing leaders to complement physician members and strengthen clinical input (Sherwood & Barnsteiner, 2017).APA , Nursing

I think these factors are changing very slowly. An analysis found that the essence of nurses and physician leaders have down disproportionally over the last ten years. Nursing leadership grew 2-6% while physician representatives grew 14-26% (Sherwood & Barnsteiner, 2017).
