Based on my health care practice as a Med Surg nurse, The scenario I considered is the Automated medication dispenser. The automated dispensing machine provides secure medication storage for patients care units along with electronic tracking of the use of narcotics and other controlled medications. Hospital and facilities have different names for their Medication dispenser depending on the manufacturing term facility where I work as a PRN staff and has also witnessed MAR ON PAPER in a smaller facility where I also did PRN at a time where medications are documented on paper. I observed with dismay how medications were being handled and documented in papers and this had a great chance for medication errors and discrepancies it was so easy to lose accountability of how and when medications were dispensed including narcotics and other controlled medications there are greater chances of administering wrong medication, wrong dosage, wrong strengths to the wrong patients and at the wrong time. Med errors can happen in big hospitals where automated dispensing machines and electronic Emar are used due to human error by uncompliant nurses who are rather too fast in carrying out their duties.  These machines have features to check and minimize these errors.

The electronic administration record offers excellent data formation and processing more than the paper MAR. EMAR is superior to paper based health record because it prevents medication errors improves clinical communication and patient outcome (Lin et al.2020). Automated dispensing machines provide computer controlled storage dispensing and tracking of medications especially narcotics and other controlled drugs , it stores accurate data and information.it provides safe dosing and screens for look alike and sound alike medication and promotes safe patient’s outcome(institute of safe med practice 2019)  Under Professionalism, the nurse leader will  use the data to advocate for patients and staff, make evidenced based decisions. These data will help to build communication and relationship management by helping to establish trust within the interdisciplinary team and sharing information for better impact.    Nursing homework help

I am currently a dialysis nurse. During my dialysis treatments, I collect various data points. These data points ultimately give an objective picture of what went on during the prescribed dialysis treatment. For example, we collect blood pressure and HR readings every 15 minutes to evaluate the patients tolerance to the treatment. We also collect data regarding the patient access pressures, and what parameters those fit into, in respect to how good the blood flow is. We collect data about the amount of ultrafiltrate obtained at the same 15 minute intervals as the access and blood pressures. Finally, we also collect data on the rate with which we remove the fluid from the patient.

Once analyzed, these data points can tell you exactly how the treatment went at different points in time, what changed and how the patient tolerated it. When a patient tolerated the treatment well, the data can be used as a testament to that. When the treatment is not tolerated, we can use the data to figure out at which point in time things strange to decline, and adjust parameters as needed, in future treatments.

Data collection and analysis is an integral part of nursing care. Trends in health can only be determine through objective data points. Nursing homework help

This aspect of nursing care is crucial to establish an evidence based practice. A clinical leader will use the data to improve outcomes for individual patients. The data collection and analysis process is dynamic and it follows all aspects of patient care. Different analysis tools are available to nurses, such as a simple plot graphs imbedded in charting software, where trends can be clearly visualized (MBA HealthGroup, 2015).

Currently, nurses spend around 5-7% of their time collecting vitals from patients (Nagle et. al, 2017). In the future, this data will be automatically obtained, and a nurse’s skill set will be better used at analyzing the data and putting proper responses in place (Nagle et. al, 2017).

Working in the psych unit exposed me to so many patients with schizophrenia. The generally depicted chronic mental disorder is characterized by a range of symptoms that affect their thoughts, emotions, and behaviors. It is usually characterized by a combination of symptoms that can include hallucinations, delusions, disorganized thinking and speech, reduced emotional expression, social withdrawal, and cognitive difficulties.

The symptoms of Schizophrenia can be categorized into positive, negative, and cognitive functions.

  • Positive symptoms include hallucinations, delusions, disorganized thinking and speech, and abnormal motor behavior. Discussion – Schizophrenia from Patient’s Point of View Nursing homework help
  • Negative symptoms involve a decrease or absence of normal functioning. These may include reduced emotional expression, social withdrawal, diminished motivation, and difficulties with speech and communication.
  • The struggles of individuals with schizophrenia can vary, but familiar challenges include social isolation, difficulties in employment or education, and problems with self-care. Treatment approaches for schizophrenia involve medication, psychotherapy, and social support. Antipsychotic medications are commonly prescribed to manage symptoms. Psychotherapy, such as cognitive-behavioral therapy (CBT), can help individuals manage symptoms, cope with stress, and improve social and occupational functioning.
  • If I were that person, I would like to tell my nurse about my experiences, needs, and fears. I might express the need for understanding and support, as well as the importance of having a trusting and empathetic relationship with my nurse. I’ve worked at a facility where I had the chance to assist with the care of a 49-year-old resident who had schizophrenia and bipolar disorder. The patient was diagnosed with schizophrenia when she was 39 years old. She dropped out of university and never got a job. The patient lived with her parents until she was 42 years old. They admitted her into a long-term facility because they could no longer care for her. She showed some positive symptoms, such as paranoia, delusions, and perceptions that were not based on reality. She also showed some negative symptoms, such as loss of motivation, feelings of emptiness, and apathy. She was often incoherent and had a flat affect. Following her train of thought was a bit tricky. She would sometimes say that we were after her life and we wanted to kill her. She isolated herself from other people in the facility; she would also refuse to take her meals. She experienced difficulties in controlling her emotions, she had struggles with her boundaries, and she did not accept her condition. She felt terrible that she was not living everyday lives like other people. She was often on medication (Olanzapine) due to undesired weight gain and insufficient therapeutic response. If I were the patient, I would want my nurse to understand that I once lived an everyday life, and the current condition is not my choice to be schizophrenic. I would let the nurse know that I am afraid of losing myself or my life to the disease.

THE APPLICATION OF DATA TO PROBLEM-SOLVING ASSIGNMENT

In the modern era, there are few professions that do not to some extent rely on data. Stockbrokers rely on market data to advise clients on financial matters. Meteorologists rely on weather data to forecast weather conditions, while realtors rely on data to advise on the purchase and sale of property. In these and other cases, data not only helps solve problems, but adds to the practitioner’s and the discipline’s body of knowledge.

ORDER A PLAGIARISM – FREE PAPER NOW

Of course, the nursing profession also relies heavily on data. The field of nursing informatics aims to make sure nurses have access to the appropriate date to solve healthcare problems, make decisions in the interest of patients, and add to knowledge. THE APPLICATION OF DATA TO PROBLEM-SOLVING ASSIGNMENT

In this Discussion, you will consider a scenario that would benefit from access to data and how such access could facilitate both problem-solving and knowledge formation.

To Prepare:

  • Reflect on the concepts of informatics and knowledge work as presented in the Resources.
  • Consider a hypothetical scenario based on your own healthcare practice or organization that would require or benefit from the access/collection and application of data. Your scenario may involve a patient, staff, or management problem or gap. THE APPLICATION OF DATA TO PROBLEM-SOLVING ASSIGNMENT

Post a description of the focus of your scenario. Describe the data that could be used and how the data might be collected and accessed. What knowledge might be derived from that data? How would a nurse leader use clinical reasoning and judgment in the formation of knowledge from this experience? THE APPLICATION OF DATA TO PROBLEM-SOLVING ASSIGNMENT
