Nursing Plan of Care

Prioritized Nursing Diagnoses Goal Nursing Interventions
Impaired physical mobility To assist the patient in restoring mobility and functional independence 1. Assess for signs and symptoms related to stiffness and pain. Stiffness and pain will prevent the patient from being fully involved in her care.Homework

2. Assess the need for multidisciplinary care. Excessive mobility problems may require physical therapy.

3. Asses the need for assistive devices. Additional support from wheelchairs, commodes, and walkers can optimize mobility and promote independence (Mairami et al., 2018).Homework

Impaired skin integrity To maintain skin integrity 1. Monitor skin integrity, noting skin color, texture, temperature, and integrity (Fraser, 2020).

2. Perform wound care according to guidelines and orders.

3. Keep the skin dry and clean.

Risk for unstable blood glucose level To ensure the patient maintains a blood sugar level of less than 140mg/dL 1. Assess the patient for signs of hyperglycemia. Hyperglycemia occurs when the body cannot use insulin properly or the body has insufficient insulin (Gioacchini, 2018).

2. Assess eating patterns. Failure to adhere to dietary guidelines can lead to fluctuations in blood glucose.

3. Assess drugs taken regularly. Some drugs can cause fluctuations in blood glucose.
