CASE STUDY: Caregiver Role Strain: Ms. Sandra A.
Sandra, a 47-year-old divorced woman, received a diagnosis of stage 3 ovarian cancer 4 years
ago, for which she had a total hysterectomy, bilateral salpingo- oophorectomy, omentectomy,
lymphadenectomy, and tumor debulking followed by chemotherapy, consisting of cisplatin
(Platinol), paclitaxel (Taxol), and doxorubicin (Adriamycin). She did well for 2 years and then
moved back to her hometown near her family and underwent three more rounds of secondline chemotherapy. She accepted a less stressful job, bought a house, renewed old friendships,
and became more involved with her two sisters and their families. Evidence-based Rationales Assignment
Sandra developed several complications, including metastasis to the lungs. Then she could no
longer work, drive, or care for herself. She had been told by her oncologist that there was
nothing else that could be done and that she should consider entering a hospice. She met her
attorney and prepared an advance directive and completed her will. She decided to have
hospice care at home and, with the help of her family, set up her first floor as a living and
sleeping area. She was cared for by family members around the clock for approximately 3 days.
Sandra observed that she was tiring everyone out so much that they could not really enjoy each
other’s company. At this time, she contacted the Visiting Nurse Association (VNA) to seek
assistance. Her plan was to try to enjoy her family and friend’s visits. After assessment, the VNA
nurse prioritized her problems to include fatigue and caregiver role strain. Other potential
problem areas that may need to be incorporated into the care plan include anticipatory grieving
and impaired comfort.

unable to do her own activities of daily living any longer.  Evidence-based Rationales Assignment

Population Health Assignment

Population Health Assignment

Population health has gained popularity in the modern healthcare system as the focus of healthcare providers expand their approach to care to include other health components such as primary prevention, social health determinants, and health equity, that affect the health of populations. The population health focus involves improving the health of all the individuals in a population, including their mental and physical health as well as general well-being by reducing health inequalities and disease occurrence, improving healthcare access, and working with other stakeholders and communities (Stanhope & Lancaster, 2019).

The population health approach can be used to transform the healthcare delivery system by coordinating healthcare services, applying the population health approach to clinical practice, health promotion, and disease prevention, and embracing digital transformation. First, the coordination of population health and clinical care can be used to find solutions to problems faced by high-risk populations and those without health insurance by developing programs targeting these groups (Shahzad et al., 2019). Clinical services in community health centers can be coordinated with rehabilitation and counseling services to address the diverse needs of local populations to reduce the need for patients to seek these services from other distant facilities. Care coordination is essential in addressing the various components of population health as no one single entity can address the needs of all patients alone. Thus, the involvement of different stakeholders such as healthcare professionals and community organizations is essential in improving the outcomes of local populations. Population Health Assignment

The population health focus can be applied to clinical practice to improve the health of populations by allowing healthcare professionals to share information about the health of populations to improve their decision-making. Information about the specific health problems affecting a population can be shared among healthcare stakeholders to facilitate the development of community-focused strategies such as mass screening and immunization community drives to address prevalent diseases in a specific population (Shahzad et al., 2019).

ORDER A PLAGIARISM – FREE PAPER NOW

The population health approach can also be used to improve the healthcare delivery system through health promotion to enhance the prevention and management of diseases that are common in a community or population. The health promotion interventions can include education programs on specific diseases, advocating for laws to protect disadvantaged groups affected by underlying social determinants of health, and developing community health programs to address population-wide health issues. These interventions can be effective in addressing health inequalities that affect underserved groups in a community.

Lastly, the population health focus can be integrated with the digital transformation in healthcare systems as hospitals are increasingly adopting electronic health record (EHR) systems to facilitate health information exchange (Shahzad et al., 2019). Technology has become an integral part of healthcare systems and can be used to transform healthcare delivery by various stakeholders in healthcare to share population-health information with ease, assess community health needs, and also track the outcomes of the health of specific populations. The data collected in EHRs can be used to build community databases for populations affected by various social health determinants that affect their access to care such as those with various chronic health conditions. The transformation of the healthcare delivery system requires a population health-focused approach that embraces care coordination, shared decision-making, health promotion, and utilization of technology to address community-wide health problems. Social Determinants of Health (SDOH) Assignment

Social Determinants of Health (SDOH) Assignment

Applying a population health focus is instrumental in transforming healthcare delivery systems. This approach necessitates a shift from reactive, disease-centered care to proactive, holistic, and preventative care that considers the social determinants of health (SDOH). These factors include income, education, neighborhood and physical environment, employment, social support networks, and access to healthcare, all of which significantly influence health outcomes (Artiga & Hinton, 2018).

ORDER A PLAGIARISM – FREE PAPER NOW

Firstly, population health management can facilitate the implementation of value-based care models. These models prioritize patient outcomes over the quantity of services provided, ultimately improving healthcare quality while reducing costs. The shift towards value-based care involves leveraging data analytics to stratify populations based on risk, using telehealth and remote patient monitoring to provide proactive care, and coordinating care to manage chronic conditions effectively (Portela et al., 2020). Social Determinants of Health (SDOH) Assignment

Secondly, a population health approach promotes interprofessional collaboration and coordination. Medical providers, social workers, public health professionals, and community organizations can work together to address the multifaceted determinants of health. This level of cooperation and integrated care delivery can enhance patient engagement, improve health outcomes, and ensure a more efficient use of resources (Wang et al., 2020).

Lastly, integrating population health in healthcare delivery systems could involve the use of health technologies. Digital tools like electronic health records, telehealth, mHealth applications, and predictive analytics can be used to monitor population health trends, provide accessible care, and predict future healthcare needs. These technologies also enable the delivery of personalized and precision care, tailoring interventions to the individual’s unique genetic, environmental, and lifestyle factors.

In conclusion, the application of a population health focus can significantly revolutionize the healthcare delivery system, fostering a paradigm shift from disease-centered to health-oriented care. Emphasizing value-based care models, interprofessional collaboration, and health technology adoption, this approach is positioned to generate impactful outcomes. By placing the emphasis on holistic health considerations and societal determinants of health, healthcare can become more proactive, efficient, and patient-centered. Ultimately, these transformative strategies aim not only to improve the health of individuals but also to achieve health equity across diverse populations, leading to a healthier society overall. These potential underscores the critical need for healthcare systems to integrate population health perspectives into their strategies, further solidifying the integral role of population health in shaping the future of healthcare delivery. Instructions

For this assessment, imagine you are a member of a Quality Improvement Council at any type of health care system, whether acute, ambulatory, home health, managed care, et cetera. Your Council has identified that newly hired nurses would benefit from comprehensive training on the importance of nursing-sensitive quality indicators. The Council would like the training to address how this information is collected and disseminated across the organization. It would also like the training to describe the role nurses have in accurate reporting and high-quality results.

The Council indicates a recording is preferable to a written fact sheet due to the popularity of audio blogs. In this way, new hires can listen to the tutorial on their own time using their phone or other device.

As a result of this need, you offer to create an audio tutorial orienting new hires to these topics. You know that you will need a script to guide your audio recording. You also plan to incorporate into your script the insights you learned from conducting an interview with an authority on quality monitoring and the use of technology to collect and report quality indicator data.

You determine that you will cover the following topics in your audio tutorial script:

Introduction: Nursing-Sensitive Quality Indicator

  • What is the National Database of Nursing-Sensitive Quality Indicators?
  • What are nursing-sensitive quality indicators?
  • Which particular quality indicator did you select to address in your tutorial?
  • Why is this quality indicator important to monitor?
    • Be sure to address the impact of this indicator on the quality of care and patient safety.
  • Why do new nurses need to be familiar with this particular quality indicator when providing patient care? Nursing-sensitive Quality Indicator Assignment

Collection and Distribution of Quality Indicator Data

  • According to your interview and other resources, how does your organization collect data on this quality indicator?
  • How does the organization disseminate aggregate data?
  • What role do nurses play in supporting accurate reporting and high-quality results?
    • As an example, consider the importance of accurately entering data regarding nursing interventions.

After completing your script, practice delivering your tutorial several times before recording it.

Additional Requirements

  • Audio communication: Deliver a professional, effective audio tutorial on a selected quality indicator that engages new nurses and motivates them to accurately report quality data in a timely fashion.
  • Length:8–10 minute audio recording. Use Kaltura to upload your recording to the courseroom, or provide a working link your instructor can access.
  • Script:A separate document with the script or speaker’s notes is required. Important: Submissions that do not include the script or speaker’s notes will be returned as a non-performance.
  • References:Cite a minimum of three scholarly and/or authoritative sources.
  • APA: Submit, along with the recording, a separate reference page that follows APA style and formatting guidelines. For an APA refresher, consult the Evidence and APApage on Campus.

Context

The American Nursing Association (ANA) established the National Database of Nursing Quality Indicators (NDNQI®) in 1998 to track and report on quality indicators heavily influenced by nursing action.

NDNQI® was established as a standardized approach to evaluating nursing performance in relation to patient outcomes. It provides a database and quality measurement program to track clinical performance and to compare nursing quality measures against other hospital data at the national, regional, and state levels. Nursing-sensitive quality indicators help establish evidence-based practice guidelines in the inpatient and outpatient settings to enhance quality care outcomes and initiate quality improvement educational programs, outreach, and protocol development.

The quality indicators the NDNQI® monitors are organized into three categories: structure, process, and outcome. Theorist Avedis Donabedian first identified these categories. Donabedian’s theory of quality health care focused on the links between quality outcomes and the structures and processes of care (Grove et al., 2018).

Nurses must be knowledgeable about the indicators their workplaces monitor. Some nurses deliver direct patient care that leads to a monitored outcome. Other nurses may be involved in data collection and analysis. In addition, monitoring organizations, including managed care entities, exist to gather data from individual organizations to analyze overall industry quality. All of these roles are important to advance quality and safety outcomes. Nursing-sensitive Quality Indicator Assignment

DISCUSSION: PATIENT PREFERENCES AND DECISION MAKING

DISCUSSION: PATIENT PREFERENCES AND DECISION MAKING

Changes in culture and technology have resulted in patient populations that are often well informed and educated, even before consulting or considering a healthcare need delivered by a health professional. Fueled by this, health professionals are increasingly involving patients in treatment decisions. However, this often comes with challenges, as illnesses and treatments can become complex. DISCUSSION: PATIENT PREFERENCES AND DECISION MAKING

What has your experience been with patient involvement in treatment or healthcare decisions?

In this Discussion, you will share your experiences and consider the impact of patient involvement (or lack of involvement). You will also consider the use of a patient decision aid to inform best practices for patient care and healthcare decision making.

1. (Chelsea).  My professional development goal is to become a knowledgeable and ethical nurse practitioner who eliminates the gaps in care for patients and improves their health. This directly correlates with Walden’s emphasis on social change. Social change is the use of complex processes to “promote the worth, dignity, and development of individuals, communities, organizations, institutions, cultures, and societies. Positive social change results in the improvement of human and social conditions.” (Walden University, 2023). To reach my professional developmental goal and develop my portfolio, I will use the following strategies: completing continued education courses, attending workshops and conferences, and getting certifications related to my area of practice. The continued development of a professional portfolio “encourages application and integration of knowledge, helps students recognize performance gaps, fosters student development, and promotes a student’s responsibility for learning” (McMillan et al., 2014). A professional portfolio will truly represent the practitioner it belongs to. (Casey & Egan, 2013). To conclude, developing one’s professional portfolio will help one reach their professional development goals.  Ethical Nurse Practitioner Assignment

2. (Whitney)

Two strategies I plan to implement for including my academic activities and accomplishments in my professional development goals is to start developing my portfolio early. I plan to start working on gathering all the pieces for it now. Burns (2018) encourages pieces and documents for the portfolio to be gathered as it becomes available. I plan to keep it updated and add new pieces to it at the start of each class to show my learning process through my MSN journey. I will keep it up to date as I go throughout the program so I do not have to go back through at the end to locate everything and just throw it together at once, which could cause me to skip important pieces to be included. Another strategy I plan to implement is by joining more associations and keeping up to date on those.  as it is a very important piece to include in my professional portfolio.  I will continue to work on my networking skills as another way to strengthen my portfolio. Hannans & Olivo (2017), state, “Networking has always been important to professional growth, and the digital world makes it easy (p. 49). These goals align with Walden University’s emphasis on social change because the goal of this portfolio will help me as I graduate and enter my FNP role. Walden (n.d.), states it, “defines positive social change as a deliberate process of creating and applying ideas, strategies, and actions to promote the worth, dignity, and development of individuals, communities, organizations, institutions, cultures, and societies” (para. 4) Ethical Nurse Practitioner Assignment

Paragraph One: What went well for me for this simulation was recognizing the signs of a stroke and knowing the interventions when a stroke is called. I have had patients with prior strokes and had a patient have a stroke in front of me at work, for which we had to intervene immediately. I feel like this scenario is mostly realistic with a couple exceptions. With the patient who is having an active stroke, sometimes the symptoms of the stroke will wax and wane. For example, I had a patient on my unit who had just recently had a baby and her husband was talking to her at bedside and noticed the patient become unresponsive. We called a Stroke Alert and by the time we had gotten the patient to the CT (approximately 10 minutes after symptom onset), the patient was responsive and able to communicate with us again. I don’t know if the scenario could have portrayed that or not,  but realistically, symptoms of stroke can vary and change throughout the assessment. Nursing homework help

Paragraph Two: Next time I would spend less time trying to gather the information I already had from the patient again. There were some assessments that I performed that were unnecessary and because of that, I ran out of options to complete my neuro assessment. I need to improve on prioritizing certain assessment questions during the focused assessment.

Paragraph Three: I learned about the NIH Stroke Scale. I honestly did not know about that assessment. I am more versed in BEFAST. In my job, it is rare, but not impossible, to have a stroke patient, considering my patients are all postpartum mothers and newborns. However, due to the rarity of the occurrence of strokes on my unit, it is important to brush up on the assessment skills. I will definitely be brushing up on my NIH Stroke Scale questions so I am more prepared for the next stroke I witness. Hopefully, that won’t happen for a while, if ever again.
